Título:Maternal preferences for consistency of complementary foods in Guatemala.

Resumen:Increasing the nutrient density of comlementary food mixtures is a common strategy for improving child nutrition in developing countries. Such modification, however, typically increases the viscosity of the mixtures, which may not appeal to caretakers or children. To assess maternal preference for complementary food consistency, 46 rural Guatemalan mothers, eachof whom had a child between 6 and 14 months of age, were interviewed by trained data collector and participated in focus group discussions. Strong opinions regarding consistencies of complementary foods were identified, which varied according to the child's age and health status. Mothers preferred thinner complementary foods for children less than one year old and the thicker foods for children more then one year old. When the child had a cough or fever, most mothers prefer thin, liquid complementary foods. When the child had diarrhoea, about half of the mothers believed that thinner complementary foods would replace the water the child lost with diarrhoea, whereas other mothers believed that thicker complementary foods would harden the stool or stop diarrhoea. This information will help guide efforts to develop improved complementary foods, particularly those for use during illness in underprivileged populations of developing countries.(AU).

Resumen:This article is a critical synthesis of 12 papers included in this supplement. The set deals with the short- and long-term effects of improving nutrition in Guatemalan villages characterized by deficient diets, high rates of infection and pronounced growth retardation in the first 3 years of life. The data reviewed come from two studies carried out over two decades: the Institute of Nutrition of Central America and Panama (INCAP) longitudinal study (1966-1977) and its follow-up (1988-89). The longitudinal study included a nutrition intervention that improved the energy and nutrient intakes of women and preschool children. Its effects included improved birthweight, reduced infant mortality rates and improved growth rates in children <3 years of age. Growth rates from 3 to 7 years of age, similar to those of well-nourished children, were not affected by the intervention. The follow-up study was conducted when the subjects were 11-27 years old. Among the long-term effect found were greater stature and fat-free mass, particularly in females, improved work capacity in males and enhanced intellectual performance in both genders. The nutrition intervention did not, on the other hand, accelerate maturation during adolescence, as measured by skeletal age or age at menarche. It is concluded that improved nutrition in early childhood has important long-term effects in the adolescent and adult.(AU).

Título:Nutrition in early life and the fulfillment of intellectual potential.

Resumen:The effects of early supplementary feeding on cognition are investigated using data collected during two periods in four Guatemalan villages. The first was the Institute of Nutrition of Central America and Panama (INCAP) longitudinal study from 1969 to 1977 and the second was a cross-sectinal follow-up of former participants carried out in 1988-1989. The principal objective of these studies was to assess the diferential effect of two dietary supplements, Atole containing 163 kcal/682 kJ and 11.5 g protein per cup or 180 mL and Fresco containing 59 kcal/247 kJ and 0 g protein per cup, that were given to mothers, infants and young children. Performance was assessed on a battery of psychoeducational and information processing tests that were administered during adolescence. Consistent differences between groups were observed on psychoeducational tests. Subjests receiving Atole scored significantly higher on tests of knowledge, numeracy, reading and vocabulary than those given Fresco. Atole ingestion also was associated with faster reaction time in information processing tasks. In addition, there were significant interactions between type of dietary supplement and socioeconomic status (SES) of subjects. In Atole villages, there were no differences in performance between subjects in the lowest and highest SES categories. On the other hand, performance in Fresco villages was best in the highest compared with the lowest SES group. After close scrutiny of alternative hypotheses, it is concluded that dietary changes produced by supplementation provide the strongest explanation for the test performance differences observed in the follow-up between subjects exposed to Atole and those exposed to Fresco supplementation..

Resumen:To assess the long-term impact of nutritional supplementation on bone mineralization during adolescence, we studied 356 Guatemalan adolescents who participated from birth to 7 years of age in a controlled supplementation trial. Bone mineralization of the distal radius was assessed using single photon absorptiometry. Children who consumed more cumulative energy from the supplement during childhood had greater bone mineral content, bone width and bone mineral density during adolescence than those who consumed less energy. The association remained after controlling for each subject's age and gender, and for the type of supplement consumed, but became statistically nonsignificant after adjusting for weight and stature. Because intake of supplement also was associated positively with weight and stature during adolescence, it is concluded that supplementing malnourished children can have a demonstrable long-term impact on bone mineralization, but that the effects are probably not beyond those due to improvements in overall somatic growth associated with supplementation.(AU).

Título:Early nutritional supplementation and skeletal maturation in Guatemalan adolescents.

Resumen:The effect of early childhood nutritional supplementation on skeletal maturation at adolescence was investigated in 663 rural Guatemalans, aged 11-18 years. Skeletal maturation was assessed by the Tanner-Whitehouse-2 method. The subjects were former participants in the Institute of Nutrition of Central America and Panama logitudinal study of growth and development (1969-77) residing in four villages (two large and two small) in eastern Guatemala. The villages were randomized within pairs to receive either a high energy, high protein supplement (Atole) or a low energy supplement with no protein (Fresco). Skeletal maturity was observed across all villages to be delayed significantly relative to a British reference for boys <14 years of age, but not for older or for girls <14 years of age. Delays in girls >14 years could not be determined reliably because many had reached maturity. Girls <14 years from Atole villages were more advanced in skeletal maturity than similar age girls from Fresco villages but these differences were found only in comparisons of the large villages. The relationship between early nutrition and biological maturation at adolescence may be obscured in this sample by the advanced age at which the subjects were examined in adolescence.(AU).

Resumen:Retrospective data on age at menarche were collected for 832 Guatemalans 15-30 years in age to test whether exposure to a high energy and high protein supplement (Atole: 163 kcal/682 kJ and 11.5 g protein per cup or 180 mL) during childhood led to earlier menarche than did exposure to a low energy, no protein supplement (Fresco: 59 kcals/247 kJ in 180 mL). Mean age at menarche was similar in Atole (13.75 +- 1.22 years; mean +- SD) and Fresco (13.74 +- 1.36 years) groups. The corresponding value for immigrants (n=144), subjects not exposed to the supplements, was 13.55 +- 1.20 years. Year of bith as well as socioeconomic status (SES) were associated with age at menarche. Age at menarche declined by 0.69 years over the 15 years period and menarche occured earlier in higher SES households. Significant positive interactions between supplement type and SES and between supplement type and year of birth were found, but plausible explanations for them could not be advanced.(AU).

Título:Nutritional supplementation during the preschool years influences body size and composition of Guatemalan adolescents.

Resumen:Effects of supplementary feeding during early childhood on body size and composition at adolescence are examined in a population with marked growth failure in the first 3 years of life. The data came from a supplementation trial conducted in rural Guatemala from 1969 to 1977 and a 1988-89 follow-up study of the same subjects at adolescence. Two pairs of villages participated in the trail. One village from each pair received a high protein-energy supplements (Atole), which significantly improved dietary intakes, whereas the other village of the pair received a low-energy, no protein supplement (Fresco), which did not impact appreciably on dietary intakes. Children from Atole villages grew better during the preschool period than children from Fresco villages. At adolescence, subjects from Atole villages were taller, weighed more and had greater fat-free masses than subjects from Fresco villages. Differences in height at adolescence were slightly reduced in magnitude relative to differences at 3 years of age. However, differences in weight were incrased in adolescence relative to 3 years of age.(AU).

Título:The effect of caloric supplementation on selected milk protective factors in undernourished Guatemalan mothers.

Resumen:The level and avidity indices of specific antibodies against tetanus toxoid, Escherichia coli 06 and apool of 10 common E. coli 0 antigens, as well as the concentration and daily output of lactoferrin and total secretory IgA (SIgA), were evaluated in the milk of moderately undernourished mothers who were in a random blind divided into two groups and given different calorie supplementations. Group A received a high calorie supplement (500 kcal/d), and group B received a low calorie supplement (140 Kcal/d). Determinations were done using ELISA in various modifications, except for lactoferrin, which was quantified by single radial immunodiffusion. The avidity indices were investigated as an evaluation of the antibody quality. In all the parameters evaluated, the only difference found between the two groups at the end of the supplementation period was in the content of total SIgAm which was kiwer in group B, both in concentration and daily output. However, The SIgA remained within the normal range. Increases as well as decreases in the levels of specific IgA antibodies ocurred within both groups. Avidity was decreased in group B only against one of the antigens tested. We conclude that moderate undernutrition does not impair the levels of milk antibodies, and supplementation does not enhance them but prevents the decrease in the content of total milk SIgA. There is a suggestion that the avidity of certain antibody specificities could be hampered.(AU).

Resumen:Uno de los esfuerzos internacionales para combatir la desnutrición más antiguos, han sido los programas de alimentación materno infantil (PAMI). El propósito del estudio que aquí se comenta fue detectar los problemas que afectan la operación del PAMI del Ministerio de Salud de El Salvador, e identificar medidas viables de aplicar a la mayor brevedad. El estudio comprendió el análisis de: a) la operación desde la selección del beneficiario hasta la entrega de la ración; b) la evolución de los beneficiarios y c) la percepción del PAMI por las madres beneficiarias. El estudio cubrió 40 Servicios de Salud seleccionados en todo el país; la revisión de 556 expedientes de niños; y entrevistas a 136 madres. Se encontró que la enfermera es quien juega el rol principal en el PAMI; que el criterio más usado para el ingreso y egreso, es la condición nutricional de los beneficiarios; que se da prioridad a los niños sobre las embarazadas; que para la mayoría de los niños se tiene control de crecimiento y vacunas, y que se supervisa principalmente el movimiento de alimentos y de beneficiarios. El estudio de expedientes de los niños reveló que la edad promedio al ingreso es de 18 meses y al egreso de 24, con una permanencia en el programa de cinco meses; y que el retardo en peso-para-edad al ingreso y el egreso fue similar para el grupo de niños estudiados, aunque fue diferente al analizarlo por permanencia y por grupos de edad. La opinión de las madres sobre el PAMI permitió entrever lo que ocurre a nivel del hogar con los alimentos donados, no obstante, es un aspecto que amerita ser profundizado. Los resultados del estudio confirman algunos hallazgos encontrados en otros; sin embargo, al ser específicos para este PAMI, permitieron la toma oportuna de decisiones sobre medidas correctivas para el mismo.(AU).

Resumen:En 1994, el Instituto de Nutrición de Centro América y Panamá (INCAP/OPS) realizó una investigación operacional para: 1) evaluar los aspectos funcionales de los principales programas de alimentación complementaria de Guatemala; y 2) generar recomendaciones orientadas a mejorar los aspectos funcionales que se encuentran débiles. Las instituciones que participaron en el estudio fueron CARE, Catholic Relief Services (CRS) y SHARE International de Guatemala. Funcionarios de estas instituciones colaboraron en el diseño de la investigación operacional para evaluar su programa, en la elaboración de un Modelo Operativo e instrumentos para la recolección de datos y en la discusión e interpretación de los resultados. La presente monografía es un estudio de caso el cual describe la experiencia del Programa CARE, ilustrando el uso y aplicación de la metodología de investigación operacional en programas de alimentación complementaria. Se definió el Modelo Operativo del sistema a estudiar, el cual incluyó los siguientes componentes: 1)Abastecimiento de los alimentos; 2)Almacenamiento; 3)Selección de beneficiarios y 4)Distribución de alimentos. Además se incluyeron las siguientes actividades paralelas: 1)Monitoreo dl Crecimiento; 2)Educación Alimentario-nutricional y 3)Supervisión...(AU).
